A 51-year-old male was diagnosed with BRVO (ischemia) with secondary neovascularization in his left eye.
Ultra-wide-field (UWF) B-scan showed vitreous opacity and several epiretinal hyperreflective signals in his left eye (fig.1 B-scan).
UWF full-range 38mm width OCTA (montage of 2 24mmx20mm) revealed a large non-perfusion area in the supratemporal quadrant with shelter signal of vitreous hemorrhage.
A large amount of neovascularization could be observed in the vitreous slab (fig.1). Photocoagulation therapy was performed on his left eye.
Meanwhile, a tiny area of angiotelectasis near the supratemporal branch vein was also found in the UWF full-range OCTA image of his right eye (fig.2 red circle).
The thickness of the whole retina, flow density, and length density of the superficial retinal vessel complex all decreased corresponding to the angiotelectasis (fig.2 & gif.).
Both structural and flow quantifications suggested that his right eye was at high risk of BRVO.
A 400kHz speed UWF full-range swept-source OCTA (BMizar, TowardPi Medical) was used in this case to visualize details of vascular alterations in each layer based on an accurate segmentation algorithm.
