A patient presented with a 320° lens dislocation in his vitrectomized eye (aqueous-filled), accompanied by a 45° posterior tilt. A lens extraction through corneal incision was performed successfully under the guidance of full-range SS-iOCT using a digital navigation surgical microscope (FOcean, TowardPi Medical).
During surgery, without zonular stability and vitreous support, the dislocated lens tilted precariously toward the vitreous cavity (fig.1). Under real-time iOCT navigation, complete en bloc removal of the dislocated lens was achieved (fig.2, 3). Throughout the procedure, iOCT delivered uninterrupted high-resolution imaging of the anterior chamber and the corneal incision, enabling continuous monitoring of chamber stability, and instrument–tissue relationships.
